1. Call 1-800-748-7001 to place the order over the phone or visit their website (you will be prompted to create an online account)

2. Provide Health Warehouse with your clinic/doctor’s information. Health Warehouse can assist in contacting your clinic for a prescription to fill an order for a 2580R Sleep Remote. You can also contact your doctor directly and have them send the prescription to Health Warehouse via:

3. The price for a replacement remote is $299 plus shipping. Following your purchase of the remote, you may be able to submit the following reimbursement codes to your insurer: L8681 and/or C1787. Please note that Health Warehouse cannot accept these reimbursement codes. They need to be submitted to your insurer
   after your remote purchase.

4. Please have your generator serial number (starting with NCR, AIR, or TML) ready when placing your order. Health Warehouse will mail the remote directly to the patient for use. The remote does not need to be programmed at the physician’s office.
