MINNEAPOLIS, MN – March 4, 2019 – Inspire Medical Systems, Inc. (NYSE: INSP) (“Inspire”), a
medical technology company focused on the development and commercialization of innovative and
minimally invasive solutions for patients with obstructive sleep apnea, announced today that five new
Blue Cross Blue Shield Association (“BCBSA”) healthcare plans have issued positive coverage policies
of Inspire therapy. These plans include: Independence Blue Cross; Blue Cross Blue Shield (“BCBS”) of
Louisiana; Wellmark; HealthNow New York; and BCBS of Kansas City, which, in the aggregate, provide
coverage for approximately 8.5 million members.

As to these positive coverage policies:

  • Independence Blue Cross covers approximately 4.0 million members across Pennsylvania and took
    effect today;
  • BCBS of Louisiana covers approximately 1.8 million members and took effect February 20, 2019;
  • Wellmark, an independent licensee of BCBSA, covers approximately 1.4 million members across
    Iowa and South Dakota and took effect January 1, 2019;
  • HealthNow New York, an independent licensee of BCBSA, covers approximately 800,000 members
    and will take effect April 1, 2019; and
  • BCBS of Kansas City covers approximately 500,000 members and took effect March 1, 2019.

There have now been 13 positive coverage policies issued by BCBSA healthcare plans following BCBSA
Evidence Street’s January 7, 2019 report, which stated that there is sufficient evidence to determine that
Inspire therapy results in a meaningful improvement in net health outcomes for patients meeting specified
criteria. These 13 Inspire therapy coverage policies comprise approximately 25 million members
currently covered under the BCBSA healthcare plans.

“We continue to work with many payers across the U.S. to develop positive coverage policies of Inspire
therapy,” said Tim Herbert, President and Chief Executive Officer of Inspire Medical Systems. “The
specific plans we are announcing today are in important target geographic areas where there are a
significant number of implanting physicians. These positive coverage policies will now provide enhanced
access to Inspire therapy, and we remain committed to making Inspire more accessible to more patients in
the future.”
